WHAT DOES THE ANNUAL MEMBERSHIP COVER?
As a patient of THE PERSONAL PHYSICIAN, you will receive 24 hours a day, seven days a week access to Dr. Campbell and his staff. Minor emergency care, bi-annual physicals, office visits, specialty referrals, and preventative and wellness consultations are all included. As part of the biannual preventive care physicals, Dr. Campbell provides each patient with a detailed written report and a personal wellness plan to be implemented throughout the year. The annual fee may be paid monthly, quarterly or annually and a variety of payment methods are available.

DO I STILL NEED TO MAINTAIN AN INSURANCE PLAN?
Yes. Your annual fee is for access to Dr. Campbell 24 hours a day, seven days a week and preventive care services that are not covered by insurance. Patients will need to maintain a PPO, POS, Medicare or major medical plan to cover services provided outside of our office, such as referrals and access to specialists, hospitalization, emergency room care, laboratory work, radiology and prescriptions. Additionally, our office is able to work with you concerning pre-tax dollars, Medical Savings Accounts, and other coverage plans.

CAN I EXTEND COVERAGE TO FAMILY MEMBERS, SUCH AS SPOUSE OR CHILDREN?
Every effort will be made to accommodate you and your family. Children (age 1 and over), spouses and partners can be added as long as there is space available in the practice.

WHAT IF I HAVE AN EMERGENCY OR NEED HOSPITALIZATION?
Dr. Campbell is available 24 hours a day; however, if you have a life-threatening emergency, call 911 and then Dr. Campbell. In the event of an emergency, Dr. Campbell will meet you at the hospital and ensure complete, thorough and timely care. If you are hospitalized, Dr. Campbell will coordinate and oversee your care with the on-staff physicians and medical personnel.

HOW LARGE IS DR. CAMPBELL'S PRACTICE?
While most General Practitioner offices have over 3,000 registered patients, THE PERSONAL PHYSICIAN is committed to a practice of no more than 175 families (350 patients). Because of the exclusivity of his practice, Dr. Campbell is able to provide immediate treatment of illness and extensive preventative care with a new focus on individualized attention and lifestyle development.

WHAT IF I NEED DISEASE MANAGEMENT, A SPECIALIST OR SURGERY?
You and Dr. Campbell will make joint decisions concerning referrals to specialists for evaluation and treatment of chronic conditions. Dr. Campbell has created relationships with leading specialists at the top of their fields and provides direct access to these physicians. Additionally, Dr. Campbell will follow your care with any specialist through careful monitoring and personal management of chronic diseases such as asthma, cancer, depression, diabetes, heart disease, high cholesterol, hypertension, or other ailments.
